Intake Form

Once we receive your intake documents they will be forwarded to our intake committee. If we think your Lab is a fit for our program we will set up a time to do a behavioral assessment on your Lab. This includes evaluating to make certain the Lab is all or mostly lab and evaluating the Lab’s behavior around food, people and other items. If we do not have a volunteer close to you, you may be asked to drive to a specified location to have your Lab assessed or you may have to wait until we have a volunteer for the assessment. Based on what we find during the behavior assessment we will determine if your Lab is a fit for our program and which of our foster homes is best suited to your Lab.
 
We are a non-profit organization, as such, we require that Labs are up to date on vaccinations, and heartworm testing. We appreciate a donation when a Lab is surrendered to us.
 
Tips about the form, picture and process:

  • L.E.A.R.N. gets several intake requests per week. We ask that you follow our procedure. Incomplete forms will either not be processed or will significantly delay the process as we try to follow up with you to fill in the blanks.
  • As you can see, L.E.A.R.N. has a detailed procedure for owner surrenders. Please understand that this procedure takes some time to complete. If you need to surrender your Lab immediately, today or within the next 48 hours, we are not the route you should take.
  • The time of our intake procedure varies depending on the number of Labs we currently have in foster care, the number of Labs waiting to come into L.E.A.R.N., the number of active foster homes we have, and the particular needs of your Lab. Therefore, we are not an immediate solution for you needing to re-home your Lab.
  • L.E.A.R.N. makes every effort to respond to every intake request whether we can take the Lab into our program or not. If you submit an intake request and have not heard from L.E.A.R.N. you can follow up with us via email at intakes@labadoption.org.
  • Please be as factual as possible. We understand that this is an emotional time, but telling us that you have a great Lab is not as informative as telling us facts about why you believe your Lab is great.
  • L.E.A.R.N. does take Lab mixes into our program. The mixes, however, must be extremely labby in appearance and behavior.
  • If you have more than 1 Lab you are looking to surrender. Please fill out 1 form per Lab.
  • If your Lab is being treated or has any ongoing medical issues please be sure to describe those on the form. Include how the issue affects the Lab’s daily life, and the treatment details.
  • Please be sure to describe why you are surrendering your Lab and give as many details as possible.
  • If you are emailing L.E.A.R.N. to follow up on your intake request, please be sure to include your name, city, state, and Lab’s name in the email. Due to the volume of requests L.E.A.R.N. is working on at any given time it is hard to identify a request with only partial information.
